Thank you for your consideration of Kaiser Permanente Southern California’s Residency and Fellowship programs.
Per National Residency Matching Program (NRMP) policy Section 6.2, please reference the following information regarding institutional and program policies for appointment eligibility to a training position.

Residency
Our Training programs accept applications via the AAMC’s Electronic Residency Application Service (ERAS). All applications must include
the following documents:- MyERAS application
- Curriculum Vitae
- Personal Statement
- Medical School Transcript
- Medical School Performance Evaluation (MSPE)
- USMLE Transcript Step 1 and 2 (when available. COMLEX Transcript, Level 1 and 2 (when available)
- Three letters of recommendation* – one of which should be from specialty in which you are applying
– Emergency Medicine applicants must submit at least one Standardized Letter of Evaluation (SLOE) as part of your letters of recommendation
– Internal Medicine [categorical] applicants must submit one letter of recommendation from either the Chair of the Department or Medicine Clerkship Director plus two additional letters of recommendation
*For PGY2 position, a letter from your PGY1 program director is required.

Visas (which include, Temporary TN work status and J-1) can only be sponsored for hard to fill positions. However, our training programs are extremely competitive and do not meet the hard to fill criteria.

NRMP matched residents or fellows must complete the following prior to beginning training:
- Clearance of Background Check: relating to references, criminal convictions, and government sanction or exclusion.
- I-9 Employment Eligibility Form: Completion of the federally required I-9 employment eligibility form with supporting documentation.
- Educational Background, Licenses, Certifications: Verification required for the position – See Requirements by Program and PGY level

Immunization requirements:- Proof of 2 TB Tests (2 step or 2 within the last 2 years.)
- Chest x-rays (if you have had a positive TB test), if needed are acceptable if no more than 1 year old
- MMR – Measles, Mumps, Rubella, Varicella, and Tdap proof of vaccinations or titer. Hep B Numerical titer required
- Proof of current Flu Vaccine (Flu Season starts November 1st)
Application Requirements:
- Copy of Official Board Scores (USMLE or COMLEX): Please upload this as a Supplemental Document (This is required for all rotations). USMLE is preferred
- Official Transcripts: Uploaded by your Medical School
- Statement of Intent: Required by the following departments- Family Medicine, Geriatrics, Sports Medicine, Internal Medicine, ICU, Pulmonary, Psychiatry (Please write two to three paragraphs expressing your interests in the specific elective you are applying for.)
- Malpractice/Insurance: A minimum of 1,000,000 per occurrence and 3,000,000 annual aggregate required. This information will be provided by your school
- Letter of Good Standing
- BLS cert (must be through the American Heart Association) – Signed or Digital Cert
- Copy of fit test certificate for N95 mask dated within a year of your rotation start date

External Resident Rotation
If you’re in a residency program in the United States and want to see what Kaiser Permanente Southern California is really like behind the scenes, why not apply for a clinical rotation with us? You’ll find a wide variety of clerkships and electives at many Kaiser Permanente Medical Centers near you.
Application Process:
The rotation application and supporting documents for clinical rotations should be received by the Physician Education department at least 30 days before the desired rotation date. Once a rotation date is approved and granted, all required compliance training modules and attestation forms should be submitted to the Physician Education department prior to the start of the rotation. Rotations may not start on a holiday or weekend if resident has not registered at the Physician Education Department.

- Current CV or copy of ERAS application
- Letter of Agreement or Good Standing from Medical School Dean/Program Director stipulating malpractice and liability insurance*
- CA State Medical License-Required of American Medical Graduates after 2nd year of residency, International Medical Graduates after 3rd year of residency
- DEA Certificate- Required of American Medical Graduates at the start of 3rd year of residency, International Medical Graduates at the start of 4th year of residency
- Current BLS or ACLS Certification
- State Issued Photo ID
- Current Health Screening w/proof of PPD/MMVR (within 1yr)
*This document is not required of students/residents from Kaiser Permanente, Southern California affiliate learning institutions for core curriculum and clerkship rotations. Affiliate learning institutions with a current Memo of Understanding (MOU) or contract for clinical education can submit a list of residents approved for clinical rotation in lieu of each student submitting a rotation application and Letter of Agreement/Good Standing. Residents applying for elective rotations apart from the learning institution must submit all of the above documents.

The institution’s list of rotating students should be submitted at least three weeks before the rotation and include:
- signature of the Clerkship Director/faculty member,
- the requested dates and duration of rotation,
- a statement regarding the institutions responsibility for malpractice and liability coverage.

Before the Start of the Rotation
After residents/affiliates are approved for a clinical rotation, each rotator will be required to complete/submit the following documents before the start of the rotation. It is recommended that these documents be completed on-line before the orientation or rotation begins:

Orientation
All clinical rotators are required to check-in at Kaiser Permanente Physician/ Staff Education Department on the first and last day of the rotation. The first day of the rotation will include an orientation. The Physician Education department coordinator will make a copy of the state issued photo ID, collect any required documents including proof of health screening, and ensure the student has completed the Kaiser Permanente Compliance documents and training. Once this process is complete the rotator will be given a badge with access to the hospital campus and parking facilities.

Sign-In Procedure
All resident/affiliate rotators are required to submit a rotation schedule. The schedule must be signed by the resident/affiliate rotator and the physician preceptor at the conclusion of the rotation.

Conclusion of the Rotation
All rotators must check out of the rotation at its conclusion and return all Kaiser Permanente property, clothing, and badges.
An exit evaluation form may be requested of each rotator.

Goldsmith Scholarship
The Oliver Goldsmith, MD Scholarship for the Promotion and Advancement of Culturally Responsive Care honors second- or third-year medical/osteopathic students who have demonstrated commitment to the advancement of culturally responsive care (CRC) through community service, clinical volunteering, or research. Each awardee will receive $5,000 toward his or her education, participate in a mentoring program with a KP clinician and complete a four-to-six week clinical rotation at a KP facility.
