- Medical School Transcript
- List of rotations/curriculum completed during residency training
- Photo
- In‑Training Exam results, (optional)
- ECFMG Certification (for International Medical Graduates)

Fellowship positions
Geriatrics Fellowship- Fontana
Kaiser Permanente Southern California is among the nation’s largest and most highly regarded health care delivery organizations. As a member of our Fellowship program, you will learn to practice cost-effective, compassionate medical care — an approach indispensable to providing health care in all future settings.
The one-year fellowship program is organized to provide a superior educational experience, assists fellows with gaining the competency to manage the entire gamut of geriatric cases and serve as a consultant to other medical providers.
To participate in the fellowship, the physician must have:
Completed an ACGME-accredited Family or Internal Medicine Residency Program
Valid California Medical license at start of program
Current DEA, BLS, ACLS certificate
USMLE transcripts for Steps 1, 2 CK, 2 CS and 3 Board scores
Three Letters of Recommendation (one must be from current Program Director)
