The Regional Office of Graduate Medical Education (GME) at Kaiser Permanente Southern California (KP SCAL) is committed to fostering a safe, respectful, and inclusive clinical learning environment that supports open communication, feedback, and reporting of concerns.
Choosing to speak up matters. Submitting feedback or a concern through this form helps the GME office identify patterns, address issues when possible, and inform system level improvements to the clinical learning environment.
Reports submitted through this form are reviewed by the GME Senior Director and the Designated Institutional Official (DIO), who determine appropriate next steps based on the nature and level of detail provided.

Confidentiality and Anonymity
This form is intended for confidential reporting of concerns to the Graduate Medical Education office.
Providing your name, program, or contact information is optional. If no identifying information is submitted, the report will be treated as anonymous, and the GME office will not attempt to identify you.
Please be aware that while anonymity is respected, details included in the description of a concern may unintentionally allow for identification of individuals or programs. Regardless of whether details provided could allow identification, if no name is included, we will assume anonymity is requested.
If you choose to provide your name or contact information, your identity will be kept confidential and shared only on a need‑to‑know basis to appropriately review and address the concern.
Anonymous reports are important for identifying trends, themes, and institutional concerns that can be shared broadly for learning and improvement. However, the ability to take specific action at the program level may be limited when reports lack sufficient detail or identifying information.

Protection From Retaliation
Retaliation or intimidation against any individual who, in good faith, seeks guidance, raises a concern, or reports unprofessional conduct or mistreatment is not tolerated. Allegations of retaliation will be reviewed and addressed appropriately.
Reports that are intentionally false and submitted for the purpose of causing harm or retaliation may also be reviewed and addressed.

Choosing the Appropriate Pathway to Report a Concern
There are different reporting pathways available depending on the type of concern and the level of urgency. The guidance below is intended to help you identify the most appropriate way to report your concern and ensure it is reviewed through the right process.
If a concern involves an immediate safety risk or urgent security issue, it is important that it be reported through the appropriate security or emergency reporting process 24/7 Emergency Incident Notification Line: 1-800-311-2113. These channels are specifically designed to respond quickly and to help protect your safety and the safety of others.
Concerns related to Title IX, sexual misconduct, or other compliance matters must be addressed through the KP Compliance Hotline , as these concerns require handling through specific reporting processes.
Patient safety events or clinical incidents should be submitted through the established patient safety reporting systems Unusual Occurrence Report – Online (UOR-O) – MD UORO (kp.org), where they can be reviewed, followed up on, and used to support learning and improvement.

Review and Follow Up Process
• Reports are reviewed by the DIO and the GME Senior Director
• Reports will generally be acknowledged within 10 business days when contact information is provided
• Anonymous reports are reviewed for trends, themes, and institutional learning opportunities
• When sufficient detail is available, follow up actions may include consultation with program leadership, review through established GME processes, or system level improvement efforts
• De-identified themes and trends may be shared with GME leadership and programs to support continuous improvement of the learning environment
This anonymous reporting form is intended to support GME‑specific concerns and feedback that may not fall within these required reporting pathways, and to provide a space for sharing issues, themes, or experiences that impact the clinical learning environment.
